Create Patient Profile

The individual seeking Medical Marijuana must create a patient profile with the Pennsylvania Department of Health’s Patient and Caregivers Registry by clicking here.

Step #2

Obtain A Physical Certification

The individual seeking Medical Marijuana must obtain a physician’s certification that he or she suffers from a “serious medical condition.” The Guide’s recommended physician in Northeastern Pennsylvania for certifying a “serious medical condition” is:

Please note that Dr. Gratz does not pay any fee to the Guide to be the recommended physician. Dr. Gratz will not see any patient for purposes of certifying a “serious medical condition” until the patient has forwarded his or her relevant medical history to Dr. Gratz for his review.

Step #3

Pay for A Medical Marijuana ID Card

After obtaining a Physician certification, the individual seeking Medical Marijuana must return to the Pennsylvania Department of Health’s Patient and Caregivers Registry and pay for a Medical Marijuana ID card.
